Care management is a set of activities intended to improve patient care, disease control and self-management, reduce distress, prevent hospital admissions or re-admissions, and enhance the coordination of care for patients with multiple or more complex conditions.

Care managers are registered nurses (RNs) who are integrated with your primary care provider, team, and the Electronic Health Record (your patient chart).

Care managers provide support around self-management plans you have created with your provider and help patients and caregivers more effectively manage health conditions.
