Insurance
Please bring your insurance card with you when you come for your appointment. Your co-pay as payment is expected at the time of service. We bill most private insurances. We accept New Hampshire Medicaid, New Hampshire Kids Gold, MaineCare, and Medicare.
SVHC is part of an Accountable Care Organization (ACO) and together we offer a Medicare Shared Savings Program for Medicare patients receiving primary care services. For more information please click the link to view the Beneficiary Notice.
Sliding Fee
Federal assistance helps Sacopee Valley Health Center to provide medical and other services that would not otherwise be available or affordable to area residents.
We have a sliding fee scale based on federal guidelines that use your income and family size to determine the amount of the discount. See below for more information on our Fee Discount Program.
